Attendee understands that participation in the live portion of the SNIS 22nd Annual Meeting & Fellows Course, July 14-18, 2025 (the “Meeting”) is entirely voluntary and requires attendees to abide by any applicable rules of conduct or local, state or federal laws that may be announced at any time during the Meeting.  In consideration of the right to attend the Meeting, Attendee assumes all risks and accepts sole responsibility for any injury (including, but not limited to, personal injury, illness, disability and death), that attendee may experience or incur in connection with attending the Meeting.  While the Society of NeuroInterventional Surgery (“SNIS”) is the convener of the meeting, Attendee understands and acknowledges that SNIS has no direct control over and is not responsible for the acts or omissions of any meeting venue personnel or others involved in producing this event, or any other attendees at the meeting.  Attendee has carefully considered the risk involved and hereby releases, covenants not to sue, discharges, and holds harmless SNIS, its officers, directors, employees, agents, and representatives (the “SNIS Released Parties”), of and from any and all Damages (including reasonable attorney’s fees) arising out of or relating thereto.

By completion and submission of the meeting registration form, I certify that I have read and fully understand this Liability Waiver and Assumption of Risk agreement and understand that it affects my legal rights. I agree and acknowledge that this waiver and release (1) shall be binding on me, my heirs, family, estate, representatives, and assigns and (2) is intended to be as broad as permitted under applicable law, and each waiver or provision herein is severable and shall apply notwithstanding the invalidation of any other provision herein.
